CEA and PSA are two of the most commonly ordered tumor markers, and a raised value on the report often causes alarm. This guide explains what CEA and PSA are, how to compare a result with the reference range, and why an elevated value is not a cancer diagnosis. What are CEA and PSA?
CEA (carcinoembryonic antigen) is a blood marker that can be raised in several cancers, but it also rises in many benign conditions. CEA is not specific to any single cancer site, and an elevated value is associated with both malignant and many non-malignant conditions (StatPearls). Because of its low sensitivity and specificity, CEA is not recommended on its own for routine cancer screening or diagnosis. It is used mainly for monitoring, for example detecting recurrence after colorectal cancer surgery (StatPearls).
PSA (prostate-specific antigen) is a marker of prostate tissue. PSA is specific to prostate tissue but not specific to cancer: both benign and malignant conditions raise it (StatPearls). Its specificity for cancer is only about 9% to 33% depending on age and cut-off, meaning most elevated PSA results are due to non-cancer causes (StatPearls). In fact, up to 86% of men with benign prostatic hyperplasia may have an elevated PSA (StatPearls).
The key point, and it bears repeating: a CEA or PSA value above the reference range does not mean you have cancer. These are markers to place in clinical context, follow as a trend, and interpret with a clinician.
How to read the result, step by step
Reading a tumor marker result is a task of recognition and interpretation. The steps below help you understand the number, while the final conclusion rests with the treating clinician.
Step 1: Identify which marker was measured
Check whether the report shows CEA or PSA. The two markers serve different purposes and use different thresholds, so knowing which one you are reading comes first.
Step 2: Confirm the unit
CEA is usually reported in µg/L, which is numerically equivalent to ng/mL. PSA is usually reported in ng/mL. Misreading the unit leads to misreading the number, so check the unit before you interpret.
Step 3: Compare with the lab’s reference range
Compare the value with the reference range printed on the report itself. For CEA, the normal upper limit in non-smokers is at or below 3.0 µg/L, and in smokers this limit is higher, up to 5 µg/L (StatPearls). For PSA, the traditional threshold in common use is at or below 4.0 ng/mL, and some settings use age-specific reference ranges (StatPearls). Reference ranges vary from lab to lab, so give priority to the range printed on your own report.
Step 4: Place the result in clinical context
A single value is not enough to draw a conclusion. Because both CEA and PSA rise in many benign conditions, the trend over time and the clinical context matter far more than a single value. A clinician combines the result with symptoms, history, examination, and other tests to interpret it.

Common mistakes
- Reading an elevated CEA or PSA value as “already having cancer.” Both rise in many benign conditions and are not a cancer diagnosis (StatPearls, StatPearls).
- Forgetting benign causes that raise PSA, such as benign prostatic hyperplasia, prostatitis, digital rectal examination, urologic instrumentation, recent ejaculation, or hard bowel movements (StatPearls).
- Forgetting that smoking and hepatobiliary dysfunction can raise CEA and cause false positives (StatPearls).
- Relying on a single value instead of the trend over time. For tumor markers, the trend and context matter more than a single value.
- Using CEA to screen for cancer. CEA is not recommended on its own for screening or diagnosis (StatPearls).
- Ignoring the lab’s own reference range, which varies from place to place.
When to see a doctor
An abnormal or unclear CEA or PSA result should be interpreted by a clinician in your specific context. A single value, even one above the reference range, is not the same as a cancer diagnosis, and diagnosis is the clinician’s job. If you receive an elevated tumor marker result, do not draw your own conclusions. Bring the report to a doctor to have it explained alongside your symptoms, history, and other tests, and to let the doctor decide whether monitoring or further evaluation is needed.
