How to Read an ANA Test: Titers, Patterns, and What Positive Means

An antinuclear antibody (ANA) result is one of the most misread lines on a lab report, because “positive” reads like a verdict when it is not one. This guide explains what the ANA test measures, how to read the titer and the staining pattern, and why a positive result on its own does not diagnose an autoimmune disease. It is written for orientation, not for diagnosis.

What is the ANA test?

Antinuclear antibodies are antibodies directed against components of the cell nucleus. They are one of the few specific disease markers used in diagnosing systemic autoimmune disorders, a group that affects 3% to 5% of the general population (StatPearls).

The single most important fact when reading an ANA: the test is not highly specific. Between 20% and 30% of the general population has detectable ANA titers, and it is the higher titers that are characteristic of people with connective tissue disorders (StatPearls). “Positive” and “diseased” are not the same statement.

On method, indirect immunofluorescence (IIF) on HEp-2 cells is the gold standard for detecting ANAs. The American College of Rheumatology affirmed this in a study published in 2010 (StatPearls).

How to read an ANA result step by step

Reading the result in a fixed order keeps you from stopping at the word “positive.”

Step 1: Check which method was used

Look at the method printed on the report. Indirect immunofluorescence on HEp-2 cells is the gold standard and returns two pieces of information: a titer and a staining pattern (StatPearls). Other assays may return only positive or negative without either.

Step 2: Read the titer, not just the word “positive”

The titer is the highest dilution at which the serum still reacts, written as 1:80, 1:160, 1:320. Dilutions of 1:160 and higher have been suggested as a helpful threshold for distinguishing potentially true-positive results from false-positive ones (J Clin Med 2025). The higher the titer, the more likely the result is a true positive, because the share of healthy people who test positive falls as the dilution rises.

One point that is easy to miss: the StatPearls review of ANA does not state any fixed titer cut-off (StatPearls). The thresholds used in practice come from other sources and depend on context, so no single number should be treated as an absolute boundary.

Step 3: Read the staining pattern

The pattern suggests which nuclear antigen is being targeted, which in turn directs the next test. A homogeneous pattern points to dsDNA, histones, or nucleosomes. Coarse-speckled points to anti-Smith antibodies. Fine-speckled points to anti-SSA/Ro and anti-SSB/La. Discrete speckles point to centromeres in cells undergoing interphase. Nucleolar speckles point to Scl-70, and speckled cytoplasmic staining points to Jo-1 (StatPearls).

Step 4: Follow up with specific antibodies

ANA is a screening test; the specific antibodies carry the diagnostic weight. Anti-dsDNA and anti-Sm are specific for systemic lupus erythematosus (StatPearls), with anti-dsDNA specificity often above 95% and anti-Sm specificity of 96% to 98% (J Clin Med 2025). Scl-70 is highly correlated with scleroderma, while anti-centromere antibodies are moderately correlated. Anti-Jo1 antibodies are associated with 30% of patients with polymyositis and dermatomyositis, and anti-Ro/SSA and anti-La/SSB titers are associated with Sjögren syndrome (StatPearls).

Step 5: Interpret in clinical context

The presence, titer, and specificity of ANAs should always be interpreted in relation to other laboratory results and the patient’s clinical symptoms, and a positive result does not always confirm a diagnosis or correlate with disease severity (J Clin Med 2025). Assembling those pieces is the clinician’s work.

  • Reading “ANA positive” as a diagnosis. Between 20% and 30% of the general population has detectable ANA titers (StatPearls).

  • Applying one fixed titer cut-off to every laboratory and every context, when the threshold depends on assay and population.

When to see a clinician

A positive or unclear ANA result should be interpreted by a clinician in your specific context. Because a meaningful share of healthy people also test positive, looking the result up and concluding you have an autoimmune disease is an easy way to be wrong. Bring the report to your clinician so it can be read together with your symptoms, history, and other tests, and so the decision about further specific antibody testing rests with them.
